The 3D HealthPathways, developed for general practice teams in the Wairarapa, Hutt Valley and Capital & Coast DHBs, has reached its 200 live pathways milestone.
HealthPathways is a “one stop shop” for best-practice, condition-specific guidelines and associated information. The online resource is designed for primary health care practitioners to use during consultation, helping them manage and refer their patients to the most appropriate specialist, hospital or community-based services. This not only helps patients get the right care, but greatly improves relationships between the people involved.
The local pathways are based on a similar programme developed by Canterbury DHB. This has reduced waiting times by enabling general practitioners to perform more procedures in the clinic. As a consequence, hospital specialists had more time to deliver more complex procedures. The initiative delivers on the refreshed New Zealand Health Strategy aim of providing health care closer to home.
All registered health practitioners working in Wairarapa, Hutt Valley and Wellington region can access HealthPathways, using the URL http://3d.healthpathways.org.nz. One-click access through MedTech or the local DHB Intranet is also available.
