The Community Health Service (CHS) provides specialised health care and support to help people live independently and have more control over their own health.
To get help from the CHS, a person must have complex health needs that would risk getting worse without the expertise and help of a registered nurse to manage then. Your GP or other registered health professional must provide referrals to CHS, so contact your family doctor for further information.
Home and Community Support Services
Home and community support services are provided by Access Community Health and Nurse Maude across the greater Wellington region. Both these organisations are well established and respected home and community support care providers.
What does this mean for you?
If you were receiving services from Access Home Health, they may have been transferred to Nurse Maude. This change will not affect the level of care you have received and you should have been informed of this change.
District Nurses
The Community Health professional will arrange a time to visit patients in their home or see them at a clinic nearby. Our services cover:
- Assessment and care of complex wounds including specialist wound treatment.
- Continence assessment
- Stoma care - support, management and equipment to enable self care.
- Oncology care
- Home Help (short term)
- Meals on Wheels (short term)
- IV Therapy (short term)
- Assessment for short term personal cares while recovering from an acute illness
It is your responsibility to ensure safe access to your property, a safe environment and that dogs are tied up or locked away when the nurse visits. Please provide a smoke free atmosphere for our nurses.
Cancellations
Please notify us as soon as possible if you are unavailable for a visit or unable to attend a clinic appointment.
If you do not notify the office, the nurse will attempt to contact you one more time. If this is unsuccessful, you will be
discharged to your GP's care.

Discharge Plan
You will be discharged when you have been assessed as no longer requiring our service. You will be advised of the final visit date and follow up care plan.
